Will it hurt? Yes. But only for a little in the beginning.
Each spike responds to tension in your body, so more tension = more sensation. The initial “Ouch!” you feel activates your body’s pain response and tells it to send happiness hormones and pain-relief chemicals to the area you’re targeting. If those spikes aren’t sharp enough—or you’re beyond relaxed already—you won’t feel much.
After a few minutes, the tingling sensation fades into a deep, relaxing warmth. The redness is purely a circulation boost. You might even fall asleep.

I'm really pleased with my Shakti pillow. I've suffered with TMJ pain for a long time, and lately with related sub-occipital muscle pain. The malleable nature of the pillow means that I can fold it or place something fimer behind it to add focused pressure on particular areas. Within minutes you can feel the increase in blood flow, and after 20 minutes I have found a significant decrease in pain; I've found it more effective than otc medication for head muscle pain. I would definitely recommend this product.

I get the most benefit from this by positioning the spikes directly under my neck and upper back. Also feels good on scalp, but I don't think you get enough pressure that way.

Also, its size has been enlarged to target the neck and shoulder area. However, if you don't feel the pressure the same way, you can try placing a rolled towel beneath The Pillow to get firmer pressure. I hope this helps you get the required pressure.
