When you place it under your head, neck and shoulders, it disperses pressure avoiding any pinch points. Your shoulders and neck experience a deep release that transitions up toward the top of your skull.

Will it hurt? Yes. But only for a little in the beginning.
Each spike responds to tension in your body, so more tension = more sensation. The initial “Ouch!” you feel activates your body’s pain response and tells it to send happiness hormones and pain-relief chemicals to the area you’re targeting. Incredible For Shoulder Tightness!
I have so much tension in my shoulders that I can never seem to fully release with an acupressure mat alone. This pillow is incredible! It covers your whole neck and shoulders and molds to your body. It's even great for lower back pain or your stomach! A must-buy!
